CVS Health works to increase value-based health care access in South Carolina

inVio Health Network and CVS Health’s Accountable Care Organization are partnering on a program aimed at improving the quality and health of 60,000 Medicare beneficiaries.

InVio Health Network and CVS Accountable Care Organization, a division of CVS Health,  announced a collaboration to participate in an innovative Medicare model through the Center for Medicare & Medicaid Innovation. The new Accountable Care Organization Realizing Equity, Access, and Community Health program focuses on reducing barriers to care by providing coordinated, patient-centered care to improve the overall health, clinical outcomes and satisfaction of patients with traditional Medicare.

InVio Health Network, one of the largest physician-led, clinically integrated networks in the Southeast, and CVS Accountable Care will leverage their understanding of the inVio communities’ health needs and their commitment to delivering care through provider enablement to improve patient and community health outcomes. The collaboration will help strengthen care coordination for patients, using advanced analytics as well as key value-based care and population health management services, programs and resources to enhance longitudinal care management.
