CVS Health's Accordant gets NCQA case management accreditation

Accordant, a CVS Health company, has received a three-year case management accreditation from NCQA. The accreditation is awarded to organizations that demonstrate compliance with NCQA's case management program standards.

CVS Health said Accordant's integrated case management program provides services on behalf of health plans, employers and third-party administrators to members with complex and chronic diseases. This is the second consecutive three-year NCQA case management accreditation for Accordant.

"At Accordant, we are pleased to receive this important accreditation from NCQA, which further demonstrates our focus on providing complete patient care management and support to our members living with complex and hard-to-manage conditions, such as rheumatoid arthritis, ALS, inflammatory bowel disease and certain cancers," Accordant vice president and general manager Ahmed Hassan said.

Accordant has more than 300 specially trained nurses who help to manage the entire patient through proactive outreach, comorbidity and symptom management, emotional and social support, and transportation coordination. As part of its case management program, Accordant manages care transitions and identifies and prevents potential problems to help members avoid costly emergency room visits and extended hospital stays.

"Case Management Accreditation moves us closer to measuring quality across population health management initiatives," NCQA president Margaret O’Kane said. "Not only does it add value to existing quality improvement efforts; it also demonstrates an organization's commitment to the highest degree of improving the quality of their patients' care."
