Drug Store News' 2010 edition of the ECRM Health Care Special Report details the successes and challenges of eight different healthcare product categories that influence the retail drug store and supermarket industries: digestives, allergy/cough-cold, internal analgesics, external analgesics, eye care, ear care, first aid and foot care. As many baby boomers become caregivers, retail pharmacy has responded with a greater focus on durable medical equipment; Carex Health Brands has revamped its online presence at Carex.com in an effort to appeal to more caregivers; and complete wireless management of health conditions through such products as blood glucose meters, blood pressure monitors, weight scales and pedometers moved on step closer to reality at the beginning of the year with the Continua Health Alliance's announcement of its first end-to-end connected health solution based on the Continua architecture. For more in-depth coverage of the home health care sector, see this latest installment of ECRM Home Health Care.

By all accounts, it’s been a very busy first half of the season for cough-cold manufacturers, even though the 52-week comparable period is only showing a 1.9% lift in overall sales across food, drug and mass (minus Walmart) for the period ended Jan. 24, according to Information Resources Inc. data. For the 2009-2010 season, the weekly percentage of outpatient visits for influenza-like illness peaked at the end of October 2009 at 7.7%, the Centers for Disease Control and Prevention reported. That peak was higher than the three previous influenza seasons as reported by the U.S. Outpatient ILI Surveillance Network.

With the defeat of Democrat Martha Coakley by Republican Scott Brown in the race to fill the seat of late Massachusetts Sen. Ted Kennedy, and the loss of the Democrats’ filibuster-proof majority in the Senate, the healthcare-reform bill stands on a knife’s edge. And how will pharmacies be reimbursed for dispensing generic drugs to Medicaid and Medicare patients if a health overhaul bill fails to pass? These topics, along with the issue of patent settlements vexing the branded and generic drug industries and a new GAO study showing that drug costs are soaring, are explored in depth in the latest installment of the ECRM Generics report.

Stymied by weak demand in a recessionary economy, intensifying generic competition, a relative dearth of research-and-development breakthroughs and an ongoing loss of patent protection for some of their biggest-selling pioneering medicines, branded pharmaceutical manufacturers have scrambled over the last three years to restore their once-enviable track record for product innovation and sales growth. And 2009 was no exception.

The challenging economy has undoubtedly dulled oral care sales as cash-strapped shoppers have increasingly turned to such high-value retailers as warehouse clubs and dollar stores. However, research suggests that manufacturers can help restore the segment's luster by focusing on offering the most value-added benefits at the best price.

Three Food and Drug Administration advisory committees voted on sweeping changes regarding the marketing, merchandising and formulation of acetaminophen products last month, though the proposed changes fell short of pulling combination products containing acetaminophen off the market. This ECRM section covers intricacies of the FDA committees' decision as well as external analgesics, foot care, PSE regulations and baby care.

Dietary supplements have been in the news lately, and not altogether for good reasons. Critics are crying for greater dietary supplement regulation after the recent recall of Hydroxycut, a popular weightloss aid, and a number of Food and Drug Administration warning letters to companies claiming to field supplements but making outlandish (and illegal) claims, as well as the constant claims by professional athletes that the banned substances for which they tested positive were found in supplements they were taking. However, all concerns aside, the category still is performing well and projected to continue its rising growth trajectory.

ECRM Cough-Cold & Allergy 2009
Author: Michael Johnsen
Date: March 2009

Despite all the debate over the use of cough-cold medicines in children under the age of 12, sales of cough-cold medicines didn’t do too badly last year. For the 52 weeks ended Jan. 25, sales of all cough-cold remedies — including cough drops, cough syrups, cold-allergy liquids and cold-allergy tablets — were up 10.5% totaling $4 billion across food, drug and mass outlets (minus Walmart) according to Information Resources Inc. data. Of course, the lion’s share of that growth comes courtesy of the latest allergy reliever to make its way from prescription vials to over-the-counter aisles. Factoring out the sales impact of McNeil Consumer Healthcare’s Zyrtec, which generated in excess of $331 million across its Zyrtec syrup, Zyrtec tablet and Zyrtec-D tablet, the overall cough-cold category still grew by 1.4%. Considering that’s on a base of $3.6 billion, that represents incremental growth of almost $50 million.

In dismal economic times, lower-cost product alternatives that save consumers money are supposed to thrive, or at least hold their own. But in the midst of a global financial meltdown, even the market for generic drugs is faltering. The worldwide sales growth rate of me-too medicines slowed to a crawl in the 12 months ended September 2008, rising a weak 3.6% over the previous 12-month period, IMS Health reported. “Global generics products generated $78 billion in audited sales in the 12 months through September, reflecting the changing industry dynamics that also are affecting branded pharmaceutical products,” noted the healthcare research firm in a December report.

Sales of vitamins across food, drug and mass outlets (minus Walmart) grew by a whopping 8.9% to $2.7 billion for the 52 weeks ended Nov. 30, 2008, according to data from Information Resources Inc. That kind of growth affirms an October 2008 survey released by the Council for Responsible Nutrition that suggested the majority of dietary supplement users didn’t cut back on their supplement regimens, despite the tougher economy. ?According to the survey, 51% of supplement users indicated that the economy’s ills wouldn’t force them to cut back on their supplement-purchasing habits. Survey results also showed that of the 51% who didn’t plan on cutting back their supplement routine, 13% reported supplements were an essential part of their overall wellness regimen — something they couldn’t do without.

If current trends in pharmaceutical sales are any indication, retail pharmacy leaders may have to look beyond the prescription department for big gains in store revenues this year. Hammered by a lack of new breakthroughs in research and development, tougher oversight at the Food and Drug Administration, generic competition and a worldwide economy in freefall, the U.S. pharmaceutical market is mired in an era of stagnant sales and declining expectations. And the outlook for an uptick in innovative new therapies from drug makers remains at historically low levels, according to industry experts.

?It just may be that the current economic slowdown could be providing a significant lift for purveyors of over-the-counter healthcare solutions. Because while your average Jane makes significant trade-offs here and there on the family’s entertainment budget—opting for a summer’s worth of Disney movies at the theater in place of a week’s actual visit to Disneyland—the one thing she’s not going to skimp on is her family’s well-being. So that means all tummy aches, sniffles and sneezes, scuffed knees and/or ear infections will still be treated by Dr. Mom, whose medicine cabinet will still be stocked. And if it isn’t stocked, relief is only a quick-and-close pharmacy trip away.

The home health care/durable medical equipment business is commanding a presence in drug stores, and products are now displayed front and center. Retailers are becoming aware of the huge demand for these products as the population ages and needs both the equipment to support mobility and independence, and the ability to manage diseases at home. Many retailers are hiring and training employees to work in this department and are dedicating significant shelf space to it.

It’s apparent that the pharmaceutical landscape continues to be dominated by major brand-name drugs losing patent protection and a flurry of generic launches aimed at taking whatever share of the market they can. That’s good news and bad news for pharmacy retailers; good in that the generic introductions are expected to drive huge profit gains during their initial 180-day exclusivity period, bad in that prices tend to fall away quite dramatically in the period thereafter due to the increased competition that results, depressing top-line sales growth.

It’s the start of a new year, and there’s no question that retail buyers are eager to scout out the hot new products making their way to this year’s ECRM Cosmetics, Fragrance & Bath event to be held in Destin, Fla., in late January. When asked what they would be looking for at this year’s ECRM event, buyers said they want unique products that will set them apart from the competition. There’s no doubt that retailers want to be different, whether it be through exclusive offerings, clever promotions or savvy merchandising. However, aside from looking for unique products, retailers also are keeping their eye on such key trends as natural and organic products, and skin care.

Voraciously defend. Push for government oversight (not more regulation, but better enforcement of existing regulations). Self-regulate. Publicize. Those are four initiatives tackled by CRN in the past year, in what could be described as a banner year for news in the dietary supplement arena. Long-awaited good manufacturing practices were finally passed, a law requiring serious adverse event reporting associated with dietary supplements was enacted in December and CRN began two key initiatives—the self-policing effort of dietary supplement advertising and a public relations campaign to fortify the reputation of supplements among consumers.

The Food and Drug Administration last month published the good manufacturing practices for the dietary supplement industry, most significantly requiring all dietary supplement manufacturers to test and qualify their raw ingredient sources. The GMPs could help boost consumer confidence, and consequently sales, in an industry where confidence has waned in the past few years, but it was almost anti-climactic as the industry prepares to mull through the 800-plus pages of the GMP document.

Both supplement industry advocates and lawmakers are putting into motion early this year initiatives that finally will apply a little credibility to an often-criticized industry. Two industry groupsa€”the Dietary Supplement Education Alliance and the Council for Responsible Nutritiona€”will be kicking off media campaigns this spring to better educate the press on supplement issues. And Congress in late December passed legislation requiring serious adverse events associated with nutritional supplements be reported to the Food and Drug Administrationa€”an initiative that is expected to inject a jolt of muchneeded credibility back into the industry.
